What is a New Construction?

 

The construction of a new source to include a building, structure, facility, or installation from which there is or may be a discharge of wastewater to the environment.

 



What is required for New Construction Permit Application Submittal?

 

1.      Application Review Fee

2.      Two (2) completed wastewater permit application Form 1, General Information, as required by Rule 62-620.301, F.A.C.

3.      Two (2) completed wastewater permit application Form 2A, as required by rule 62-620.301, F.A.C.

4.      Preliminary Design Report

5.      Two (2) completed Agricultural Use Plans (AUP), Form 62-640.210(2)(a), as required by Chapter 62-640 F.A.C., accompanied by a signed letter of agreement between land owner or domestic residuals hauler and residual generator.

6.      Capacity Analysis Report as required by Section 62-600.405 F.A.C.

7.      Two (2) 8-1/2 X 11” schematics of the roads leading to the wastewater treatment plant.

8.      Two (2) 8-1/2 X 11” schematics of the treatment process to include piping and tank volumes.

9.      Two (2) 8-1/2 X 11” schematics of the effluent disposal system in relation to the plant.

10.  Two (2) 8-1/2 X 11” maps of the residuals disposal site(s)

11.  Two (2) copies of reuse feasibility studies for plants with surface water discharge.

12.  An Operation and Maintenance Performance Report as required by Section 62-600.735, F.A.C.

 

 




What is a Renewal?

 

The re-certification of an existing operational license, usually issued for five (5) years.

 

What is required for a Renewal Submittal?

 

 

1.      Application Review Fee

2.      Two (2) completed wastewater permit application Form 1, General Information, as required by Rule 62-620.301, F.A.C.

3.      Two (2) completed wastewater permit application Form 2A, as required by rule 62-620.301, F.A.C.

What is a Substantial Revision?

 

A substantial revision is a change in the permit conditions in response to a modification to the facility or activity which is reasonable expected to lead to a substantially different environmental impact or which involves a substantially different type of wastewater, residuals or industrial sludge treatment, reuse or disposal system.

 

What is required for a Substantial Revision Submittal?

 

 

1.      Application Review Fee

2.      Two (2) completed wastewater permit application Form 1, General Information, as required by Rule 62-620.301, F.A.C.

3.      Two (2) completed wastewater permit application Form 2A, as required by rule 62-620.301, F.A.C.

4.      All supporting information and design to support your request.

 

 

  






What is a Minor Revision?

 

A minor revision to an existing permit is a change to the permit conditions, including any decrease or increase in staffing requirements or monitoring frequencies, correction of minor errors or typographical mistakes, transfer of a permit to a new owner, extension of compliance dates or construction schedules, delineation of outfalls or discharge points, or modification to the facility or activity that is not expected to lead to substantially different environmental impact.

 

What is required for a Minor Revision Submittal?

 

 

1.      Application Review Fee

2.      Two (2) completed wastewater permit application Form 62-620.910(9) as required by Rule 62-620.352, F.A.C.

 

   

 






What is a Transfer?

 

The “Transfer of Permit” is the formal act of transferring the ownership of a permit from the exiting permittee to a proposed permittee.

 

What is required for a Transfer Submittal?

 

 

1.      Application Review Fee

2.      Two (2) completed wastewater permit application Form 62-620.910(11) as required by Rule 62-620.340, F.A.C.
