Permit Application Notice Sign
Using the sign provided in this link, please fill out the remaining information and post this pursuant to Sec. 1-2.051(a) within 15 days of submitting an application for any of the following initial permits or initial authorizations:

   - wastewater permits in excess of 100,000 gallons discharge per day
  
- authorizations issued under section 1-7.202(1)(c) or (d), Rules of the EPC
   - authorizations issued under section 1-10.05B, Rules of the EPC
   - air construction permits issued under Chapter 1-3, Rules of the EPC**

The EPC recommends you weatherproof the sign prior to posting, as it must be maintained for at least 30 days.  If you have applied for one of the above listed authorizations/permits, you must comply with all applicable notice requirements in sec. 1-2.051.  **Please note Chapter 1-3 may require a larger sign that meets the requirements of sec. 1-2.051.
